The school year is halfway through, and I am preparing my daughter’s admission requirements for big school. One of the requirements for school admissions is to provide a Birth Certificate from Philippine Statistics Authority (PSA).
It is important for us, parents, to have a copy of our PSA birth certificates. Not only is it asked for school admissions, but also so for travel, employment, claims, marriage, loans, or vaccines. In addition, birth certificates can be used as proof of identification for kids or adults who do not have any valid IDs yet.
What is a PSA Birth Certificate?
A PSA Birth Certificate is an official document detailing an individual’s personal information. It states the full name, gender, birth date, birthplace, and parents’ details. A PSA Birth Certificate does not expire and is used to identify an individual and evaluate one’s legal capacity to enter a transaction or contract. However, note that some entities may require a recent (no later than six months) copy of the PSA Birth Certificate.
If you are a first-time parent, the hospital creates the report with the Local Civil Registry Office (LCRO), where you have given birth. The LCRO is responsible for endorsing the birth certificate to PSA for certification and printing.

When to Request a PSA Birth Certificate
If you plan to request your newborn’s birth certificate, wait for 1 to 2 months after the transmittal date for Metro Manila birthplaces and 2- 3 months after the transmittal date for provincial birthplaces.
Birth certificate processing at PSA may take longer due to the verification and digital conversion.
You can request the PSA Birth Certificate any time as needed for older children and adults.
How to Get a PSA Birth Certificate
While there is an option to personally process and request the birth certificates at the Census Serbilis Centers, I prefer the convenience of ordering online. I didn’t want to spend time on traffic and queuing when I could request it in the comfort of my home.

I want to share how easy it was for me to log a request for my daughter’s birth certificate. Let me walk you through the steps on how to request a PSA Birth Certificate through PSA Helpline.
You can order your own certificate or request on behalf of your son, daughter, or parents. However, you cannot order on behalf of your spouse. Your spouse must request his/her birth certificate by him/herself.
How to Request a Birth Certificate Online
- In the browser, type in https://psahelpline.ph/.
- Click on the Order Now button.

Click on Birth, tick the I accept the Terms and Conditions and Privacy checkbox, and click the Continue button.

Fill Up Form to Request for Own PSA Birth Certificate
- Answer the question – Are you requesting for your own Certificate of Live Birth or for someone else?
Click on the My Own Birth Certificate option, then click the Continue button.

- Prepare the following information and click on Continue
Information to be provided for the issuance of Own Birth Certificate:
- Name, Sex, Civil Status, Birthday, and Valid Government ID
- Father’s Name
- Mother’s Maiden Name
- Birthplace
- Late Registration (if applicable)
- Purpose of Request
- Legal Proceedings (if applicable)
- Delivery Address and Contact Information

Step 1. Enter your personal details in the form and then click the Continue button.

Step 2. Confirm that the name you indicated is the same as the name reflected in your Certificate of Live Birth. If you are a married woman, ensure that you have indicated your Maiden Name.

Step 3. Enter your Father’s First Name and Last Name. The Middle Name is optional.

Step 4. Enter your Mother’s Maiden First Name, Middle Name, and Last Name.

Step 5. Enter your birthplace.

Step 6. Declare Late Registration (if applicable).

Step 7. Select the Purpose of the Request.
If you are requesting for travel or foreign employment purposes, you need to indicate the country you will be traveling to or working in.

If your purpose is not listed, choose Others, and specify your reason.

Other options do not require additional information. Click on the purpose option and then click on the Continue button.

Step 8. Declare if there were any legal proceedings done.
If there is any legal proceeding done, click on the appropriate option. Then, answer the additional question, “Have you previously acquired a copy of the certificate with the annotation?”

If there are NO legal proceedings done, click on None and the Continue button.

Step 9. Enter your Delivery Address and Contact Details. Then, click on the Continue button.

Proceed to the Pay for the Birth Certificate section.
Fill Up Form to Request for Someone else’s PSA Birth Certificate
- Answer the question – Are you requesting for your own Certificate of Live Birth or for someone else.
If you are requesting on behalf of another family member, like in my case, my daughter, click on For Someone Else. Then, answer the question, “Whose Birth Certificate are you requesting?“. Then, select the option from the displayed choices. Click on the Continue button.

- Prepare the following information and click on Continue button.
Information Needed for the Birth Certificate Request (For Someone Else)
Information About the Requestor:
- Name, Sex, Civil Status, Birthday, and Valid Government ID
- Delivery Address and Contact Information
Information About the Child or Parent whose birth certificate is being requested.
- Name and Birthday
- Father’s Name
- Mother’s Maiden Name
- Birthplace
- Late Registration (if applicable)
- Purpose of Request
- Legal Proceedings (if applicable)

Step 1. Enter your personal information, then click on the Continue button.

Step 2. Enter your family member’s personal information, click on the Continue button.

Step 3. Enter your family member’s father’s information and click on the Continue button.

Step 4. Confirm the mother’s information. It is grayed out and non-editable because I have already provided the information earlier.

Step 5. Enter your family member’s birthplace details, then click on the Continue button.

Step 6. Declare Late Registration (if applicable).

Step 7. Select the Purpose of the Request.
If you are requesting for travel purposes, you need to provide the country you will be traveling to.

If your purpose is not listed, choose Others and specify your reason.

Other options do not require additional information. Click on the purpose option and then click on the Continue button.

Step 8. Declare if there were any legal proceedings done.
If there is any legal proceeding done, click on the appropriate option. Then, answer the additional question, “Have you previously acquired a copy of the certificate with the annotation?“

If there are NO legal proceedings done, click on None and the Continue button.

Step 9. Enter the Delivery Address and Contact Details. Then, click on the Continue button.

- Review all the information you entered. Next, tick on the I confirm checkbox and click the Continue button.
Note: Personal Information that you entered should be reflected on the blank spaces you see on the screenshot below.

Proceed with payment.
Pay for the Birth Certificate
After completing the form and confirming the information, select your payment option, then pay the fee.

As of writing, a copy of the PSA Birth Certificate costs 365 pesos. The fee already includes the document, service, and courier fees.

PSA Helpline sends out an email with the Order Number, request details, and payment options. If you cannot settle the payment in one sitting, refer to the email.

Once the payment is settled, the PSA Birth Certificate will be delivered within 3-4 working days for Metro Manila addresses and 3-8 working days for provincial addresses. You should receive an email notification once the PSA Birth Certificate is ready for release.

What to Prepare for Delivery
Couriers do not release the birth certificate if the requestor is not present to receive the document or present a valid ID. So, you must be at the delivery address at the time of delivery. If you have any concerns about delivery, you can contact PSA Helpline to schedule the delivery.
My PSA Helpline Experience
I have tried ordering birth certificates and marriage certificates from PSA Helpline, and all my transactions were hassle-free. I live in the greater Metro Manila area, and I received the documents within 4 days. I can attest that the documents are safely handled and that every time a document was delivered to me, the courier asked for proof of identification.

Other Tips When Requesting PSA Birth Certificate
- Double-check all the information that you enter in the form. Any errors can lead to delay or, worse, a negative certification.
- Confirm that name in the form and your ID are the same.
- Check your email for notification for release.
- Prepare to receive the document personally and keep your government ID handy.
- In case of unsuccessful delivery or delivery schedule requests, contact PSA Helpline through email or phone.
